Financial Arrangements
Our office is dedicated to
the concept that our patients should be able to keep
their teeth for a lifetime with maximum comfort, health,
and appearance. To help our patients realize this goal
we offer financing options. We understand that it is
important to stay within a dental budget, and therefore,
we will discuss fees with you in a private and
respectful environment.
Our office has a
specialized computer system that allows us to process
insurance claims electronically for both rapid and
maximum reimbursement. Please bring your insurance
information at the time of your appointment.
For your convenience our office accepts the following
methods of payment: Cash, Personal Check, Visa,
MasterCard, American Express, Discover and Citi
HealthCard.
Emergencies
Every effort is made to
structure your dental care so that emergencies are
prevented. However, we also recognize that you may have
a problem develop after hours. For after hour
emergencies call our office telephone number for
immediate assistance.
New Patient Form
This is a form we are providing as a download to our new
patients. Click on the pdf icon to download the form
then print, and fill out at your convience.
Once you have filled out the New Patient Forms, please either mail, see address below, or fax, 706.698.3383, them to us so that we may complete your patient file. If you are unable to mail of fax the New Patient Forms, please arrive to your appointment 15 minutes early so that we may have time to enter your information into your file before you are seen.
